Urgent – IMMEDIATE action needed. Phone call required (e.g. safety, security or environmental risk). High – Fix needed within 1 week (e.g. something that has not yet failed but will within days). Medium – Fix needed within 1 month. Low – Fix needed within 3 months.
Provide a clear description of the location of the fault on site including identifiable details such as GPS reference, closest equipment operating sign, name of building, etc.
